    Check their phone’s compatibility

  2. If device is compatible check DUO Admin to see if there is a phone listed but not added correctly > Remove phone > Help them add it again

  1. Last 4 of SSN/ITIN is NOT verified or unavailable, you need to verify ALL three of the following

    1. MMDD of Birth

    2. Postal address on file with SJSU

    3. Email address on file with SJSU
